Expired Sessions

You have twenty minutes to fill out the form and submit it, otherwise you will be redirected to the main page when your session expires, then you will have to repeat the process gain. This is an intentional security feature.

Browser issues

Use Google Chrome or Microsoft Edge for best results. Firefox is an old depreciated browser and it's not recommended. If you are using Firefox and you have ANY issues, use Edge or Chrome.

Maintenance, Installation, MAC

A maintenance trouble is defined as a problem where something WAS working correctly and now it is not. An installation is a brand new system that has to be installed. A MAC is a MOVE, ADD, or CHANGE to an existing customer's service.

Form Fields

ALL displayed fields are REQUIRED. You will not be able to submit the form if you do not fill in all fields.

You must select the priority, then the product type. If for some reason you are redirected back to the main form, you will need select the priority again by changing it from the current value to another, then select the product type again. This is not a bug it's intentional.

A Project Manager is required for all new system installations. A Project Manager is NOT required for upgrades or MAC work. A new system installation is a brand new system at a customer's location.

If you do not have the information required to complete one of the fields on the form, use your best judgement, and enter something that makes some sort of sense.
